The gate opening from the common area onto the street was chained and locked shut;  on or about 10/20/13.     EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM IN THE ACCORDANCE WITH 29 CFR 1903.19
