29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    (a)  The employer did not develop and implement a lockout tagout program that addresses how to control hazardous energy, (e.g. mechanical and pneumatic) when performing servicing and/or change-overs on equipment, such as but not limited to the Flexo and Koppers print/score/slotters.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  (a)  Lockout/tagout procedures were not developed to control hazardous energy (e.g. mechanical and pneumatic) when machine operators performed servicing/change-overs on the Flexo and Koppers print/score/slotters. Operators had to remove machine guards to wash down rollers and the equipment remained in an energized state during the tasks.

29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o):    (a) On the Flexo Curioni printing/slotting/folding machine near the corrugate discharge conveyor, pulleys were unguarded and exposed employees to in-running nip points and entanglement hazards.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  (a)  The employer did not develop a written hazard communication program describing how chemicals will be labeled, material safety data sheets will be maintained, and the information/training will be conveyed to employees. Employees work with chemicals, such as but not limited to Flex-Open Fast Dry (ink) and Wisdom Adhesives.
