29 CFR 1910.272(e)(1): The employer did not provide training to employees at least annually and when changes in job assignments would expose them to new hazards in the grain handling facilities: Employees hired from a temporary labor service were exposed to hazards associated with grain handling such as but not limited to amputations, fires and explosions, chemical exposure and inhalation in that the employer failed to train these employees before putting them to work performing grain handling operations.

29 CFR 1910.272(e)(2): Employees assigned special tasks in the grain handling facility, such as bin entry and handling of flammable or toxic substances, were not provided training to perform these tasks safety: On or about September 24th, 2013, employees hired from a temporary labor service were exposed to hazards such as grain engulfment, amputations, and fire and explosion in that the employer failed to train these employees on the procedures and methods for safe bin entry prior to sending them into grain bins.
