S. Public Health Service.

Public Health Service current at the time these evaluations and procedure took place:    a) Throughout the facility, on or about 9/15/13, A licensed practical nurse completing the 3 shot hepatitis B vaccination series on 7/15/13 was not provided with test for the hepatitis B surface antigen 1 -2 months following the third and final vaccination of the series.    b) Throughout the facility, on or about 9/19/13, A licensed practical nurse completing the 3 shot hepatitis B vaccination series on 8/19/13 was not provided with test for the hepatitis B surface antigen 1 -2 months following the third and final vaccination of the series.
