29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:Note:  Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard.  On or about September 25, 2013 and times prior thereto employees were exposed to fall heights of approximately 50 feet.  To access the top of bin #23 employees climb over an obstruction on the catwalk.  While climbing the obstruction the employee is above the guard rail.

29 CFR 1910.303(g)(1)(i)(A): For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the depth of the working space in the direction of access to live parts (as measured from the live parts where they were exposed or from the enclosure front or opening where they were enclosed) was less than indicated in Table S-1 of Subpart S of Part 1910:     On or about September 25, 2013 and times prior thereto the area in front of the electrical starters and breakers for the product conveyors was obstructed by the parking of vehicle.  The depth of the working space was approximately 27 inches where 36 inches are required to provide safe access.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):  On or about September 25, 2013 and times prior thereto, employees were exposed to the hazards of electrical shock from the exposed conductors of the conveyor starters and breakers where the front covers had been removed.
