At the 327 Roxy Lake job site, Poplar Bluff, MO, on 9-26-13, inspections were not conducted to prevent an employee from operating an excavator in an unsafe condition.  The excavator was operated on rip rap on a slope at approximately 27 to 29 degrees, and where the excavator operator lost control of the machine resulting in it sliding down the rock, before he was able to return the machine to the top of the embankment.
