29 CFR 1926.102(a)(1): Employees shall be provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents.  WORKSITE:   1. The employee cutting a hole at the roof peak was not wearing eye protection.  2. The employees installing sheathing using pneumatic nailing devices was not wearing eye protection.   3. The employee who was blowing debris off the roof using an electric air blower exposing employees who were not wearing any eye protection to injury.

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.   WORKISTE: The employees engaged in roofing activities on the main roof (6/12 pitch) and the addition roof (7/12 pitch) were not protected from the hazard of falling between 9 feet 9 inches and 21 feet to the ground by the use of appropriate fall protection devices/systems.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.  WORKSITE: The employees, exposed to fall hazards while performing roofing activities on the steep pitched roof of a single family house were not provided with training in the recognition of hazards associated with falls or the procedures necessary to eliminate of minimize those hazards.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support.  WORKSITE:   1. The side rails of the Louisville model L2121-28, 28 foot aluminum extension ladder extended only one step above the roof eave and the ladder was not adequately secured to prevent accidental displacement.   2. The side rails of the Lynn 24 foot aluminum extension ladder extended only two steps above the rake edge of the roof, and the ladder was not adequately secured to prevent accidental displacement.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders shall be used at an angle such that the horizontal distance from the top support to the foot of the ladder is approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support).  WORKSITE: The Lynn 24 foot aluminum extension ladder, used by employees to access the front roof, was setup at an angle where the horizontal distance from the top support to the foot of the ladder exceeded one-quarter of the working length of the ladder.

29 CFR 1926.1053(b)(10): The top of a non-self-supporting ladder shall be placed with the two rails supported equally unless it is equipped with a single support attachment.  WORKSITE: The Lynn 24 foot aluminum extension ladder, used by employees to access the front roof, was setup in such a manner that the side rails were not equally supported and no single attachment device or other safe means were used, exposing employees to a 15 foot fall.

WORKSITE: The employees, manually carrying rolls of underlayment, bundles of shingles and full sheets of plywood onto the roof were unable to maintain at least three points of contact while using the following ladders at all times:    1. The Lynn 24 foot aluminum extension ladder used to access the front roof.   2. The Louisville model L2121-28, 28 foot aluminum extension ladder used to access the back roof.
