29 CFR 1904.29(b)(2): The employer did not fill out or correctly fill out an OSHA Form 301 or equivalent for each recordable injury or illness:   At the Facility - On or about 9/17/13, an OSHA Form 301 or its equivalent was not filled out for the following work-related injuries or illnesses occurring in the past 6 months:       a)   An injury received on 6/13 when an employee in the floor department tripped and fell to knees resulting in 2 days away from work.       b)    An incident in the compounding area on 9/13 resulting in two days away form work due to a sore throat.      c)    A second incident in the compounding area on 9/13 resulting in two days away from work due to a sore throat.      d)    An injury in the compounding area involving no lost or restricted time due to an employee getting omeprazole in right eye.
