29 CFR 1926.451(c)(1)(iii): Ties, guys, braces, or outriggers shall be used to prevent the tipping of supported scaffolds in all circumstances where an eccentric load, such as a cantilevered work platform, is applied or is transmitted to the scaffold. Location: 101 Sixth Ave, New York, NY 10019 In front of Building. a) Employees assembled a baker scaffold that only had the outrigger braces at one side. The baker scaffold was free standing and when an employee applied eccentric loads (extension ladder weight and body weight) to the baker scaffold the scaffold tipped over. An employee fell approximately 16 ft to the ground below; on or about 9/27/13.

29 CFR 1926.454(c): When the employer has reason to believe that an employee lacks the skill or understanding needed for safe work involving the erection, use or dismantling of scaffolds, the employer shall retrain each such employee so that the requisite proficiency is regained. Retraining is required in at least the following situations: Location: 101 Sixth Ave, New York, NY 10019 In front of Building. a) Employees assembled a baker scaffold that only had the outrigger braces at one side. The baker scaffold was free standing and when an employee applied eccentric loads (extension ladder weight and body weight) to the baker scaffold the scaffold tipped over. An employee fell approximately 16 ft to the ground below. Scaffold safety re-training is required; on or about 9/27/13.
