29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:  On or about 9/24/2013, during residential roofing activities, the employer did not ensure that employees were provided fall protection while tearing off and installing roofing material on a 6/12 slope roof at a height of 9.9 feet.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about 9/24/13, during roofing activities, the employer did not provide a training program and training to employees conducting roofing activities of fall hazards, the procedures, and techniques to be followed to minimize their fall hazard exposure.
