29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii).  a)  Work Site: 545 Kearny Avenue, Kearny, NJ  Employee was walking/working on a single pump jack scaffold plank, thereby exposing the employee to an approxumate 18 foot fall hazard.  Violation observed on or about 10/2/2013.   b) Work Site: 545 Kearny Avenue, Kearny, NJ  Employee was walking/working from tubular framed scaffold that was not fully planked, as only 3 of 6 planks were in place, thereby exposing the employees to an approximate 18 foot fall hazard.  Violation observed on or about 10/2/2013.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  a)  Work Site:  545 Kearny Avenue, Kearny, NJ  Employees were exposed to falls of approximately 18 feet to the ground below as no methods of fall protection were used on the tubular welded frame scaffold.
