Violation observed on or about 10/2/2013. b) Work Site: 545 Kearny Avenue, Kearny, NJ Employee was walking/working from tubular framed scaffold that was not fully planked, as only 3 of 6 planks were in place, thereby exposing the employees to an approximate 18 foot fall hazard. Violation observed on or about 10/2/2013.
