Aisles and passageways shall be kept clear and in good repairs, with no obstruction across or in aisles that could create a hazard.     a)     On or about October 03, 2013, in the rear storage area, center of the floor, the employer did not ensure that a circular (in diameter) piece of pertruding steel in the floor was covered or removed, thereby exposing employees to a potential trip/fall hazard.  b)     On or about October 03, 2013, in the front/side area of the store, by the greeting cards, the employer did not ensure that floor did not have a raise crack across the floor asile, thereby exposing employees to a potential trip/fall hazard.

29 CFR 1910.22(d)(1): In every building or other structure, or part thereof, used for mercantile, business, industrial, or storage purposes, the loads approved by the building official shall be marked on plates of approved design which shall be supplied and securely affixed by the owner of the building, or his duly authorized agent, in a conspicuous place in each space to which they relate. Such plates shall not be removed or defaced but, if lost, removed, or defaced, shall be replaced by the owner or his agent.     a)     On or about October 03, 2013, in the rear of store area, the employer did not ensure that the managers office space, which had boxes of material stored above, was equipped with a load limit plate, thereby exposing employees to a potential struck-by hazard.  b)     On or about october 03, 2013, in the rear of store area, the employer did not ensure that the receivers office space, which had boxes of material stored above, was equipped with a load limit plate, thereby exposing employees to a potential struck-by hazard.

However, panelboards other than the dead front externally-operable type are permitted where accessible only to qualified persons.     a)     On or about October 03, 2013, in the rear, first floor storage area, by the rear door exit, the energized electrical panal (Box 101) had a missing circuit breaker on the front of the electrical panel, thereby exposing employees to an electrocution hazard.  b)     On or about October 03, 2013, in the rear, first floor storage area, electrical panel number P-157 had missing circuit breakers on the front of the electrical panel, thereby exposing employees to an electrocution hazard.  c)     On or about October 03, 2013, in the rear, first floor storage area, electrical panel number P-158 had missing circuit breakers on the front of the electrical panel, thereby exposing employees to an electrocution hazard.
