29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: (a) On or about October 3, 2013, the path to ground for the Hilti hammer drill and the extension cord was not permanent and continuous in that one of the two extension cords it was plugged into was missing the grounding prong plus the hammer drill attachment plug was damaged such that the grounding prong could not make good contact as it was shorter than the other pins of the plug.
