29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:    a) Worksite - An employee was framing a roof from a pallet on the forks of a JLG Model # 10054 SkyTrak Telehandler, Serial # 0160051349, approximately 23 feet 2 inches above the ground; on or about 10/04/13.     Note:  In addition to abatement certification the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.

29 CFR 1926.602(c)(1)(viii)(A): The industrial truck was not equipped with a safety platform/cage firmly secured to the lifting carriage and/or forks when used for lifting personnel:  a) Worksite - An employee framing a roof was lifted to his work position on an unsecured pallet mounted on the forks of a JLG Model # 10054 SkyTrak Telehandler, Serial # 0160051349, approximately 23 feet 2 inches above the ground; on or about 10/04/13.  Note:  In addition to abatement certification the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a) Worksite - An employee framing an approximate 8-12 pitch roof approximately 23 feet 2 inches above the ground was not protected from falling; on or about 10/4/13.  Note:   Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.
