29 CFR 1910.217(c)(1)(i): It shall be the responsibility of the employer to provide and insure the usage of "point of operation guards" or properly applied and adjusted point of operation devices on every operation performed on a mechanical power press. See Table O-10. a. Located in the facility, employees were required to operate a 55 ton part-revolution mechanical power press, model number 55, serial number 10167, in order to stamp metal material that had an adjustable hand crank on a pullback system that would allow for adjustments up to 12 inches on the pullback restraints. The pullback system was not properly applied which allowed an operator to come into contact with the point of operation, thereby exposing the operator of the press to an amputation hazard.

29 CFR 1910.217(d)(9)(i): The employer shall establish a diesetting procedure that will insure compliance with paragraph (c) of this section. a. Located at RJM Stamping, employees were operating mechanical power presses that were set up by a die setter without any die setting procedures available for the die setter, thereby exposing the employees to the hazards associated with the operation of a mechanical power press.

29 CFR 1910.217(e)(1)(i): It shall be the responsibility of the employer to establish and follow a program of periodic and regular inspections of his power presses to ensure that all their parts, auxiliary equipment, and safeguards are in a safe operating condition and adjustment. The employer shall maintain a certification record of inspections which includes the date of inspection, the signature of the person who performed the inspection and the serial number, or other identifier, of the power press that was inspected. a. Located at RJM Stamping Company, the employer failed to document periodic and regular inspections on mechanical power presses that employees were required to operate.
