29 CFR 1910.146(c)(2): The employer did not inform exposed employees, by posting danger signs or by any other equally effective means, of the existence and location of and the danger posed by the permit spaces:    (a) The Stork C Surge Tank did not have signage to inform employees of the permit space and its dangers including the entanglement hazard created by the mixer inside the tank.    (b) Silo 29 did not have signage to inform employees of the permit space and its dangers including the entanglement hazard created by the mixer inside the silo.    (c) Silo 30 did not have signage to inform employees of the permit space and its dangers including the entanglement hazard created by the mixer inside the silo.    (d) Silo 31 did not have signage to inform employees of the permit space and its dangers including the entanglement hazard created by the mixer inside the silo.    (e) Silo 32 did not have signage to inform employees of the permit space and its dangers including the entanglement hazard created by the mixer inside the silo.    (f) Silo 33 did not have signage to inform employees of the permit space and its dangers including the entanglement hazard created by the mixer inside the silo.

(a)          The employer did not have a written certification showing all hazards in the permit space had been eliminated for the Stork C Surge Tank.   (b)          The employer did not have a written certification showing all hazards in the permit space had been eliminated for the Silo 29.   (c)           The employer did not have a written certification showing all hazards in the permit space had been eliminated for the Silo 30.   (d)          The employer did not have a written certification showing all hazards in the permit space had been eliminated for the Silo 31.   (e)          The employer did not have a written certification showing all hazards in the permit space had been eliminated for the Silo 32.   (f)           The employer did not have a written certification showing all hazards in the permit space had been eliminated for the Silo 33.

29 CFR 1910.147(c)(6)(i)(C): Where lockout was used for energy control, the periodic inspection did not include a review, between the inspector and each authorized employee, of that employee's responsibilities under the energy control procedure being inspected.    (a) Employees are not receiving an annual review of performing lockout/tagout procedures to ensure that employees are performing the procedures correctly.
