29 CFR 1926.501(b)(1): Employee on a vertical walking/working surface having an unprotected side or edge which is six feet or more above a lower level were not protected from falling by the use of a guardrail system, a safety net system, or personal fall arrest system: a) On or about 10/04/13 the employer allowed his employee to work on and from DOKA concrete forms with an incomplete fall protection system in that the employee was only provided one positioning device resulting in not maintaining 100% fall protection while moving from point to point on the forms.
