29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: On or about September 23, 2013 at Barstow 27 Well #2 of the Jetta Operating Lease, an employee was not protected from falling from the beam of a pumpjack, exposing the employee to fall hazards exceeding four feet.
