29 CFR 1926.451(a)(1): Each scaffold and scaffold component was not capable of supporting, without failure, its own weight and at least 4 times the maximum intended load applied or transmitted to it.  Navajo traffic interchange underpass bridge, Interstate 40 at Mile Post 325, East of Holbrook, Arizona: On September 30, 2013, an employee fell approximately 16 feet when he stepped onto a section of the work platform and it collapsed.  The scaffold was not capable of supporting its own weight and its intended load.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:  Navajo traffic interchange underpass bridge, Interstate 40 at Mile Post 325, East of Holbrook, Arizona: On September 30, 2013 and at times prior thereto, the work platform was not inspected for visible defects by a competent person before each work shift. At the beginning of the work shift on September 30, 2013, an employee fell approximately 16 feet when he stepped onto a section of the work platform and it collapsed. The work platform had also been previously used to support employees when they removed concrete and serpentine bars from the girders and performed grinding in preparation for the Nelson studs.

Navajo traffic interchange underpass bridge, Interstate 40 at Mile Post 325, East of Holbrook, Arizona: On September 30, 2013 and at times prior thereto, employees working on an elevated work platform were not trained in the control of hazards associated with the maximum intended load and the load-carrying capacity of the work platform.
