29 CFR 1926.451(b)(2)(ii): Where scaffolds must be used in areas that the employer can demonstrate that narrow platforms and walkways cannot be at least 18 inches (46 cm) wide, employees on those platforms and walkways were not protected from fall hazards: On or about 10/8/13, the employer did not ensure that employees working on the roof of the building were not using the basket guardrails to climb onto, and off of the roof.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems: On or about 10/8/13, during commercial steel erection activities, employees were working on the roof and not protected from fall hazards by utilizing a fall protection system/device. Northeast Erectors, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29CFR 1926.760 (a)(1), which was contained in OSHA inspection number 830381, citation number 1, item number 2 and was affirmed as a final order on 3/20/2013 with respect to a workplace located at 16750 East Lincoln Way, Dalton Ohio 44659.
