29 CFR 1903.19(c)(1):  The employer did not submit to the Agency certification demonstrating that abatement is complete for each serious violation for which the Agency indicates in the citation that such abatement certification is required.  1.  On or about January 12, 2014, the Agency (OSHA) had not received abatement certification for Citation 1, Item for inspection Number 943067 - fall protection program.  2.  On or about January 12, 2014, the Agency (OSHA) had not received abatement certification for Citation 1, Item 2 for inspection Number 943067 - use of portable ladders.  3.  On or about January 12, 2014, the Agency (OSHA) had not received abatement certification for Citation 1, Item 3a for inspection Number 943067 - fall hazard training.  4.  On or about January 12, 2014, the Agency (OSHA) had not received abatement certification for Citation 1, Item 3b for inspection number 943067 - ladder /stairway safety program.

29 CFR 1926.501(b)(13):   Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   On or about October 9, 2013, two employees were exposed to a fall of approximately nine feet without fall protection while stripping shingles on a one-story residential roof with a 4/12 pitch.   Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.1053(b)(1):   Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   On or about October 9, 2013 two employees used a portable metal extension ladder to access a 4/12 slope roof approximately 9 feet 2.5 inches above the ground. The ladder side rails did not extend at least 3 feet above the upper landing surface; they were extended less than one foot above.   Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

On or about October 9, 2013, two employees were exposed to a fall of approximately nine feet while using a ladder to gain access to a one-story residential roof with a 4/12 pitch; the ladder side rails did not extend at least 3 feet above the upper landing surface and the employees had not been trained by the employer to recognize hazards related to ladders and the procedures to be followed to minimize the hazards.   Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
