29 CFR 1926.602(b)(3): The safety requirements, ratios, or limitations applicable to machines or attachment usage covered in Power Crane and Shovel Associations Standards No.3 of 1969 and applying to cranes, machines, and attachments under this part were not complied with:    a) Bridge Over Slotznick Run on Schoolhouse Lane, Harrisburg PA - An employee was injured while operating a Link-Belt Hydraulic Excavator (Model 2700 Quantum, Serial Number E5I7-7449) after the machine tipped completely over onto its roof landing in the upside down configuration above an open trench when the manufacturers rated lifting capacity was exceeded, on or about October 10, 2013.    Abatement certification and documentation required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.  Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
