29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent):    a) On or about October 5, 2013, at the above addressed job site,  an employee was exposed to a fall hazard while directing wheeled tractors from a loading dock which was equipped with a guardrail system that lacked an intermediate rail.

29 CFR 1910.151(b): There was neither an infirmary, clinic, or hospital used for the treatment of all injured employees in near proximity to the workplace nor a person or persons adequately trained to render first aid:     a) On or about October 5, 2013, at the above addressed job site, the employer did not provide medical services and first aid for employees who were exposed to fall hazards when medical responders were located at a distance greater than a 10 minutes away from a work site.
