29 CFR 1926.503(a)(1): Employer did not provide fall protection training to those employees who might be exposed to a fall hazard.  The employer is failing to protect employees from fall hazards while engaging in residential construction activities six feet or more above a lower level.  A recent example was documented at 17715 Dorcas Circle, Omaha, NE 68130 where employees were engaged in roofing activities at heights greater than six feet even though they had not received adequate instruction on fall hazards and the means to eliminate those hazards.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladder was not used at an angle such that the horizontal distance from the top support to the foot of the ladder is approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support).  Job site located at 17715 Dorcas Circle, Omaha, NE 68130-The employer is failing to ensure that metal extension ladder used to access the working surface of the roof is set-up at an angle where the horizontal distance from the top support to the foot of the ladder was one-quarter of the working length of the ladder.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels were not protected from falling by a guardrail system, safety net system, or personal fall arrest system, or an alternative fall protection measure that meets a provision described in paragraph (b) of 29 CFR 1926.501.   The employer is failing to protect employees from fall hazards associated with re-roofing activities on an existing residential home with a roof height over 10 feet to eave from the ground level.  The most recent example of this hazard was found on a job site located at 17715 Dorcas Circle, Omaha, NE 68130 where the employer had employees re-roofing an existing residential home with ground to eave height over 10 without any form of fall protection.
