29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s).      (a) 463 Main Street, Oak Hill WV: The person designated as the competent person did not comply with the requirements of this standard in that they did not identify hazards of the job site, materials and equipment such as but not limited to, ensuring the employees wore fall protection when exposed to a fall hazard, as observed on or about 10/15/2013.      ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED      The Beckley Insulation Company, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.20(b)(2), which was contained in OSHA inspection number 313375016, citation number 1, item number 2b and was affirmed as a final order on August 1, 2011, with respect to a workplace located at Curtis Lane, Prosperity, WV 25909.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:       (a) 463 Main Street, Oak Hill WV: On October 15, 2013 employees were exposed to a fall of approximately 24 feet to the lowest portion of the ground, while conducting roofing work on a 8 in 12 slope roof. The employees were not protected by a personal fall arrest system.      ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED    The Beckley Insulation Company, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 313375016, citation number 1, item number 8 and was affirmed as a final order on August 1, 2011, with respect to a workplace located at Curtis Lane, Prosperity, WV 25909.
