Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: (a). Worksite located at 205 N. Belt Highway, St. Joseph, Missouri- Employees were exposed to fall hazard in that openings on the back side of the roof were not guardrailed, nor were the men working near the openings utilizing personal fall arrest systems.

29 CFR 1926.1053(a)(8): A metal spreader or locking device shall be provided and utilized on each stepladder to hold the front and back sections in an open position when the ladder is being used: (a). Worksite located at 205 N. Belt Highway, St. Joseph, Missouri- An employee was using a step ladder near the opening of the parapet wall in which the step ladder was leaning on the wall without the spreader bar in place. The ladder, which was being misused, could easily slide from the parapet wall to the opening where and employee could fall to his death.
