29 CFR 1910.67(c)(2)(v): A body belt and lanyard attached to the boom or basket was not worn while working on and from an aerial lift.   a) 121 Cider Mill Road, Flemington, NJ 08822. The employee was working on and from a non-insulated truck mounted aerial lift with no fall protection system in place in that the employee was not wearing a lanyard attached to the boom or basket exposing the employee to fall hazards of approximately 17 Feet.    Incident occurred on or about 10/17/13.

29 CFR 1910.268(b)(7): The employer did not ensure that no employee approached or took any conductive object closer to any electrically energized overhead power lines and parts than prescribed in Table R-2 or in (i) - (iii) of this section:   a) 121 Cider Mill Road, Flemington, NJ 08822. The employee was working in close proximity to energized power lines without appropriate insulation and or PPE.    Incident occurred on or about 10/17/13.  Employee came into contact with energized power lines while installing new communications cable.

Additionally, the employer could not demonstrate that his employee was already trained in the precautions and safe practices required by this section prior to his employment.   a) 121 Cider Mill Road, Flemington, NJ 08822. The employer did not train nor could he produce certification of employee training precautions and safe practices required when working on telecommunication line, prior to allowing employee to work in close proximity to energized power lines.    Incident occurred on or about 10/17/13.  On or about 10/17/13 employee contacted energized power lines while installing new communication cable.

Incident occurred on or about 10/17/13. On or about 10/17/13 employee was working in close proximity to energized power lines while not wearing any head protection.

The employee was working on and from a non-insulated truck mounted aerial lift in close proximity to energized overhead power lines closer than clearances set forth in table R-2 of this section.    Incident occured on or about 10/17/13. On or about 10/17/13 the employee used bucket of truck mounted aerial lift to raise telecommunication cable when he came into contact with overhead energized power lines.
