Employer failed to provide documentation that an effective hearing conservation program had been implemented.

29 CFR 1910.95(c): 29 CFR 1910.95(c)(1): A continuing, effective hearing conservation program as described in 29 CFR 1910.95(c) through (n) was not instituted when employees noise exposures equalled or exceeded an 8 hour Time Weighted Average (TWA) sound level of 85 dBA: (a) On or about November 5, 2013, a sorting employee was exposed to a dose of 67.8% (8-hour time weighted average of 87.2 dBA) for a 474 minute sampling period. (b) On or about November 5, 2013, a machine operator was exposed to a dose of 69.9% (8-hour time weighted average of 87.4 dBA) for a 480 minute sampling period.
