29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.   a) Plant and Front Office roofs -  On or about 10/04/13 workers were repairing/patching the roofs with prepared tar compounds in proximity of the unprotected borders without fall protection equipment. The workers were exposed to 22.5 ft. falls from the plant roof to ground level, 15.5 ft. falls from the office roofs to ground level, and to 7.0 ft. falls  from the plant roof to the office roof.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET)

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder  length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) Plant and Front Office roofs -  On or about 10/04/13 workers engaged in roof repair utilized a portable ladder to connect the office roof (situated at 15.5 ft. above ground level) with the plant roof (situated at 22.5 ft. above ground level). The ladder was too short and did not pass three feet above the plant roof.  Employees were exposed to 7.0 ft. falls from the plant roof onto the office roof.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET)
