29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):  On or about 10/3/2013, at the above addressed jobsite, employees were conducting carpentry work on a 5:12 residential roof (Lot 13/Block 14) and were not protected from fall hazards of approximately 12ft.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  On or about 10/3/2013, at the above addressed jobsite, an employee walking on the top of a cement wall (Lot 30/Block 12) approximately 10ft from the ground level was not protected from fall hazards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about 10/3/2013, at the above addressed jobsite, employees were conducting carpentry work on a 4:12 residential roof and were using an extension ladder that the rails did not extend at least 3' above the roof eave.
