29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.    a) United Methodist Church, 26 Jordan Street, Skaneateles NY - On or about 21 October, 2013 - Employees were working on a low sloped roof above 6 feet less than 50 feet in width, without the use of a safety monitoring system.    JD. Ferro Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 386282, citation number 1, item number 1 and was affirmed as a final order on 21 June, 2012, with respect to a workplace located at 124 West Buffalo Street, Ithaca NY.

29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupters for personal protection.  a) United Methodist Church, 26 Jordan Street, Skaneateles NY - On or about 21 October, 2013 - Employees were using an extension cord to power a circular saw that was not protected by a ground fault circuit interrupter.
