29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.95(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent: (a) On or about 10/30/13 - The Planer Feed Operator was exposed to continuous noise at 305.0% of the allowable 8-hour time-weighted average sound level of 85 dBA or an equivalent level of 98.0 dBA. The sampling was performed for 446 minutes during one shift. Zero exposure was assumed for the unsampled time of 34 minutes. (b) On or about 10/30/13 - The Moulder Feed Operator was exposed to continuous noise at 85.9% of the allowable 8-hour time-weighted average sound level of 85 dBA or an equivalent level of 88.9 dBA. The sampling was performed for 444 minutes during one shift. Zero exposure was assumed for the unsampled time of 36 minutes. (c) On or about 10/30/13 - The Multi-cut Trim Saw Operator was exposed to continuous noise at 93.4% of the allowable 8-hour time-weighted average sound level of 85 dBA or an equivalent level of 89.5 dBA. The sampling was performed for 448 minutes during one shift. Zero exposure was assumed for the unsampled time of 32 minutes. (d) On or about 10/30/13 - The Rip Line operator was exposed to continuous noise at 78.3% of the allowable 8-hour time-weighted average sound level of 85 dBA or an equivalent level of 88.2 dBA. The sampling was performed for 446 minutes during one shift. Zero exposure was assumed for the unsampled time of 34 minutes. (e) On or about 10/30/13 - The Stringer Machine(old) Operator was exposed to continuous noise at 129.9% of the allowable 8-hour time-weighted average sound level of 85 dBA or an equivalent level of 91.9 dBA. The sampling was performed for 431 minutes during one shift. Zero exposure was assumed for the unsampled time of 49 minutes. (f) On or about 10/30/13 - The Shaper Feed Operator was exposed to continuous noise at 117.7% of the allowable 8-hour time-weighted average sound level of 85 dBA or an equivalent level of 91.2 dBA. The sampling was performed for 450 minutes during one shift. Zero exposure was assumed for the unsampled time of 30 minutes.
