29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a. At the site, an employee was struck by the drill tip of a Komatsu PC 78 drill. The drill was used to re-drill a damaged hole. During the operation a worker standing less than three feet from the hole was struck in the head by the drill rod. On or about 10/18/2013
