Observed on or about 10/21/13.

29 CFR 1926.451(b)(4): Each end of a platform, which was not cleated or otherwise restrained by hooks or equivalent means, did not extend over the centerline of its support at least 6 inches:    a) Jobsite: 103 Woodridge Drive - Each end of a working platform of a fabricated frame scaffold did not extend over the centerline of its supports at least 6 inches exposing employees to fall hazards of more than thirteen (13) feet. Observed on or about 10/21/13.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.    a) Jobsite: 103 Woodridge Drive - Employees installing plywood from the second level of the fabricated frame scaffold were exposed to fall hazards of approximately thirteen (13) feet without the use of personal fall arrest systems or guardrail systems.  Observed on or about 10/21/13.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.    a) Jobsite: 103 Woodridge Drive - Employees were not provided with a safe means of access and/or egress to a fabricated frame scaffold platform, exposing employees to a fall hazard of approximately thirteen (13) feet.  Employees were using the side frame of the scaffold to access and/or egress the working levels. Observed on or about 10/21/13.
