29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) On September 26, 2013, four employees working a commercial building at 2901 E. Enterprise Avenue, Appleton, WI 54911, were not protected from falling approximately 12 feet from the eave of the roof by the use of a fall protection system or equipment. ABATEMENT DOCUMENTATION AND ABATEMENT CERTIFICATION REQUIRED.

29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person: a) A rope that was part of a personal fall system was directly tied to the anchor with no connector attached. ABATEMENT DOCUMENTATION AND ABATEMENT DOCUMENTATION REQUIRED.
