This was most recently evident during the inspection of the facility located at 125 South Broad St. Fremont, NE 68025, where the employer has flour mill employees operating and cleaning around the BK4 sifter located on the 5th floor without guarding the point of operation on the sifter created by the swiftly moving counter weights.

29 CFR 1910.272(j)(3):   Compressed air was used to blow dust from ledges, walls, and other areas in grain handling facilities when machinery presenting an ignition source was not shut down, and/or when all other known potential ignition sources in the area had not been removed or controlled:                                                                                                           The employer failed to protect employees from potential explosion hazards associated with using compressed air to clean grain handling facilities without eliminating all potential ignition sources.  This was most recently evident during the inspection of the facility located at 125 South Broad St. Fremont, NE 68025, where the employer has flour mill employees using compressed air hoses and nozzles rated at 30psi for cleaning around the third, fourth and fifth floors of the flour mill while all equipment is still operating and potential ignition sources exist.  Instances include but are not limited to:  i.    Belt and pulley assemblies on top of the sifters where potential bearing failures could create enough heat to ignite the highly flammable flour dust as it is blown into the air in its smallest particulate form.  ii.    Chain and sprocket assemblies along all of the horizontal and vertical auger systems on the third and fourth floors where a bearing failure could create enough heat to ignite the highly flammable flour dust as it is blown into the air in its smallest particulate form.  iii.   Missing light switch cover on the overhead light switch located on the fifth floor.

Snap switches mounted in boxes shall have faceplates installed so as to completely cover the opening and seat against the finished surface.     The employer failed to protect employees from potential electrical hazards associated with missing faceplates on light switches.  This was most recently evident during the inspection of the facility located at 125 South Broad St. Fremont, NE 68025, where the employer has flour mill employees working throughout the facility and the light switch for the fifth floor ceiling lights is located directly next to the door is missing the cover leaving the 110v electrical lines exposed.
