29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    272 Nashua St, Leominster, MA    On 11/22/13 and at times prior, employees operating the Blow Mold machine were exposed to the hazards of moving parts due to incomplete machine guarding in many areas of the machine.  Many of the fixed and interlocked guards provided incomplete protection due to employee ability to reach around them and access the hazard zones.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  272 Nashua St, Leominster, MA  On 11/22/13 and at times prior, employees operating the Blow Mold machine were exposed to the hazards of bottle trimming knives due to incomplete machine guarding.  The fixed and interlocked machine guards provided incomplete protection due to employee ability to reach around them to access the hazard zones.  Additionally on 9/24/13 and at times prior, interlocked guards providing protection to the trimming knives were disabled, exposing employees to amputation hazards.
