(b) At City Wide Ready Mix at the truck wash on or about October 16, 2013 the deck for the truck wash was missing a section of railing leaving a space 4 feet wide and an fall hazard of 5 feet.    (c) At City Wide Ready Mix at the cement plant on or about October 16, 2013 the deck on the concrete mixing tower was missing midrails and railing.    (d) At City Wide Ready Mix at the little silo workers had to climb on top of the silo to grease equipment and do maintenance and there was no railing to prevent falling.    (e) At City Wide Ready Mix at the main fly ash silo workers had to climb on to a beam to grease equipment and do maintenance and there was no railing to prevent falling.    (f) At City Wide Ready Mix along the main feed belt to the concrete plant workers had to change out rollers and do maintenance on the belt and rollers and did not have a railing or other form of fall protection.

(b) At City Wide Ready Mix on or about October 16, 2013 two light fixtures hung by their wiring that was held together by wire nuts and was not mounted in a safe manner or disconnected and removed from service.  City Wide Ready Mix did not ensure the safety of the equipment as required by (iv) and (viii) of this section  (c) At City Wide Ready Mix on or about October 16, 2013 a phone line was wired into an electrical service panel designed for 110 and 220 volt nominal voltage.  The phone line, which was low DC voltage by design, was not physically seperated from the higher AC voltage circuits.  City Wide Ready Mix did not ensure the safety of the equipment as require by (vii) and (viii) of this section.
