29 CFR 1926.453(b)(2)(v): A safety harness with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: a. At the site - an employee was exposed to a fall hazard, in that, the use of fall protection was not enforced while working from a JLG 400S aerial lift, serial number 85184. The working height was 27 ft. Violation observed on or about 10/22/13.
