29 CFR 1910.37(a)(4): Safeguards designed to protect employees during an emergency (e.g., sprinkler systems, alarm systems, fire doors, exit lighting) must be in proper working order at all times.  Wood Shop Area:  On or about 22 October 2013 and times prior thereto, emergency lighting in the wood shop was inoperable.  This exposed employees to smoke inhalation and/or fire hazards from delays in exiting the building.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch.  Wood Shop Area:  On or about 22 October 2013 and times prior thereto, both tongue guards on the  JET 8" Bench grinder, serial number 2090788, were not installed properly and distance of wheel to guards measured approximately 3/8 inch.  This condition exposed employees to a struck by hazard.

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.  Wood Shop Area:  On or about 22 October 2013 and times prior thereto, air nozzles provided to employees did not reduce air pressure to 30 P.S.I.  Shop air was set at approximately 100 P.S.I.   This exposed employees to air embolism/laceration hazards if they contacted the discharge to their skin.

Wood Shop and Garage Area:  On or about 22 October 2013 and times prior thereto, two locally fabricated, 120 volt, 4-plug junction box was wired to an extension cord.  This exposed employees to a 120 volt electrical shock hazard.

29 CFR 1910.305(a)(2)(ix): All lamps for general illumination shall be protected from accidental contact or breakage by a suitable fixture or lampholder with a guard. Brass shell, paper-lined sockets, or other metal-cased sockets may not be used unless the shell is grounded.  Garage area:  On or about 22 October 2013 and times prior thereto, a 120 volt, BB-2870 portable light was missing its guard. This condition exposed employees to an electrical shock and burn hazard.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket).  Garage area:  On or about 22 October 2013 and times prior thereto, a 120 volt, Porter-Cable belt sander, Model 360, serial number 099929 had a frayed cord exposing wires.  This condition exposed employees to an electrical shock and burn hazard.
