29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch. Wood Shop Area: On or about 22 October 2013 and times prior thereto, both tongue guards on the JET 8" Bench grinder, serial number 2090788, were not installed properly and distance of wheel to guards measured approximately 3/8 inch. This condition exposed employees to a struck by hazard.

Wood Shop Area: On or about 22 October 2013 and times prior thereto, air nozzles provided to employees did not reduce air pressure to 30 P.S.I. Shop air was set at approximately 100 P.S.I. This exposed employees to air embolism/laceration hazards if they contacted the discharge to their skin.
