29 CFR 1910.134(h)(2)(i): Respirators were not stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals or  were not packed or stored to prevent deformation of the facepiece and exhalation valve.   Physical or physiological stress. Employees were voluntarily using agency provided half face air-purifying respirators. Respirators were not cleaned after use. Employees stored their individually maintain respirators in personal lockers and/or tool bags. Three instances were noted. This condition exposed employees to dermatitis, inhalation or ingestion of a hazardous substance.

Anchorage Facility Maintenance, on or about 22 October 2013 and times prior; the employer did not ensure that material safety data sheets for each hazardous chemical located in the workplace was readily accessible. Random samples of chemicals were selected. Six instances were noted:       Instance (a) Clear-Seal Concrete Protective Sealer, No. 606, 1 gallon.        Instance (b) Schneider Standard Gray Spray Enamel, Q1360-9026, 12 ounce.        Instance (c) 3M General Purpose Adhesive Cleaner, 08984, 1 quart.       Instance (d) NAPA Premium Starting Fluid, 11 ounce.        Instance (e) Minwax water based polycrylic, 1 quart.         Instance (f)  Sunnyside Mineral Spirits, 1 gallon.  This condition exposed employees to chemical burns, irritation of the eyes and upper respiratory tract, and skin dermatitis hazards.

Facilities maintenance shop, garage bay; on or about 22 October 2013 and times prior thereto; three Penco pallet storage racks were not secure to the wall or floor. Pallet racks are located in a seismically active zone. Pallet storage racks did not have the maximum safe load limits posted. This condition exposed employees to a struck by hazard.

Instance (b) 2011 - OSHA Form 300; Case No 201102038000 Column (F) going to Sand Point, AK Airport, stepped on a piece of wood and twist his ankle; does not indicate which ankle was affected.            Instance (c) 2012 - OSHA Form 300A; was not certified and signed by company executive until 23 October 2013.   This condition leads to inaccurate reporting of injuries and illnesses.

Personal protective equipment observed in the workplace included to; hearing protection, safety glasses, face shield, respirators, hard hats and gloves.This condition exposed employees to unsafe work practices.
