On or about 10/10/13, railroad spur, intermediate rails were not midway between the top rails and the gangway floors of a mobile transloading unit, which exposes an employee to fall hazards from heights 10 feet above a lower level.   c.  On or about 10/10/13, railroad spur, intermediate rails were lacking at a platform of a rail tank car, which exposes an employee to fall hazards from heights 10 feet or greater above a lower level.

On or about 10/10/13, railroad spur, an acid gas respirator cartridge change schedule based on objective data was not implemented for employees who wore half mask respirators during loading of 36% hydrochloric acid, which exposes employees to chemical inhalation hazards.

29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposes an employee to injury and the defective or damaged item was not removed from service:   a.  On or about 10/10/13, railroad spur, at the electrical box of the mobile transloader unit, the outer black jacket of a flexible cord was pulled back at the attachment plug exposing the cord's insulation and covered conductors and the flexible cord lacked strain relief which exposes employees to electric shock hazards.
