29 CFR 1910.333(c)(3)(iii)(A): Vehicles or mechanical equipment capable of having parts of their structures elevated near energized overhead lines were not operated so that a clearance of 10 feet (305 cm) was maintained: (a) On 10/22/2013, On the street in the front of 57-54 Maspeth Ave. Maspeth, NY JLG lift (1250AJP) was used to raise employees for shoot the movie. The boom of the lift was seven feet to the energized overhead lines. ABATEMENT VERIFICATION IS NOT REQUIRED PURSUANT TO 29 CFR 1903.19.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): (a) On 10/22/2013, On the street in the front of 57-54 Maspeth Ave. Maspeth, NY Employees connected electrical cables to a generator for power the spot light. One of cables was frayed. ABATEMENT VERIFICATION IS NOT REQUIRED PURSUANT TO 29 CFR 1903.19.
