29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: (a)Academy Roofing, Inc. at 2002 W. 76th Avenue Building #4, Denver, CO 80221: On and before October 22, 2013, the employer did not ensure that employees were protected from falling when using a wheel barrel to dump debris into a chute. The path on the flat roof used to access the chute was within 27 inches of the edge, and no guardrails or other forms of fall protection were in place. The condition exposed employees to fall hazards of up to 30 feet.

Conditions exposed employees to fall hazards of up to 10 feet.
