29 CFR 1926.651(f): A warning system such as barricades, hand or mechanical signals, or stop logs was not utilized when mobile equipment was operated adjacent to an excavation, or when such equipment is required to approach the edge of an excavation, and the operator does not have a clear and direct view of the edge of the excavation. On or about October 23, 2013, and times prior thereto, warniing system such as barricades, hand or mechancal signals, or stop logs were not used when mobile equipment driven adjacently to an excavation by operators that do not have a clear and direct view of the edge of the excavation. This condition exposed employees to struck-by, crush or entrapment hazards.
