29 CFR 1910.303(c)(3)(i): All splices and joints and the free ends of the conductors were not covered with an insulation equivalent to that of the conductors or with an insulating device identified for the purpose:         a)  Fabrication Shop, Suite 109:  On or about October 22, 2013, employees were exposed to electrical shock hazards when using a Makita 4" hand grinder, SN 20135A, 120V, 13A with a fabricated plug spliced the cord and wrapped with masking tape.       b)  Fabrication Shop, Suite 109:  On or about October 22, 2013, employees were exposed to electrical shock hazards when using a Makita 4" hand grinder, SN 180291A, 120V, 13A with approximately a 10 inch damaged section of electric flexible cord wrapped in loose electrical tape with the original wiring cut and rewired.       c)  Fabrication Shop, Suite 109:  On or about October 22, 2013, employees were exposed to electrical shock hazards when in close proximity to a Maxx Air Velocity fan, which was spliced into 4 pieces of various electric flexible cords wrapped in electrical tape and plugged into a damaged relocatable power tap.
