29 CFR 1910.23(a)(2): Ladderway floor opening(s) or platform(s) were not guarded by a standard railing with standard toeboard(s) on all exposed sides (except at the entrance to opening) with passage through the railing either provided with a swinging gate or so offset that a person could walk directly into the opening: Employees performing maintenance and housekeeping tasks in the boot pit were exposed to fall hazards of approximately 15 feet to the bottom of the boot pit in that the ladder opening on the first platform was not protected from falls.

29 CFR 1910.27(c)(2): A clear width of at least 15 inches (0.4 m) was not provided each way from the centerline of ladder(s) in climbing space(s):Note: Does not apply where cages or wells were necessary: Employees climbing the ladder to the motor/gearbox level to perform housekeeping and maintenance tasks were exposed to fall hazards of approximately 8 feet to the next lower platform in that the oil drain pipe for the gear box sticks out approximately 10 inches into the climbing path of the fixed ladder.
