The employer was performing electrical work for the Colorado State University Animal Sciences Building remodel.  One employee was performing work including installing conduit and running wire.  The employer did not ensure that the employee used protective eyewear.  This condition potentially exposed the employee to flying object hazard.

29 CFR 1926.1101(k)(9)(ii):  Training was not provided prior to or at the time of initial assignment and at least annually thereafter for employees likely to be exposed in excess of a PEL and for employees performing Class I through IV asbestos operations:  (a) Weifield Group Contracting LLC, at 350 W Pitkin Street, Fort Collins, CO: On or about 10/10/13, Weifield Group Contracting LLC did not ensure that training was provided prior to or at the time of initial assignment for employees performing Class I through IV asbestos operations.  The employer was performing electrical work for the Colorado State University Animal Sciences Building remodel.  One employee routed conduit through a basement concrete wall penetration that also contained two pipes that had thermal system insulation (TSI).  Bulk surveys of presumed asbestos containing materials had been conducted prior to the project.  TSI in the basement had been determined to contain both amosite and chrysotile asbestos in excess of one percent.  The two pipes were located 1.5 inches and 4.5 inches from the conduit.  The TSI was analyzed and found to contain eleven percent amosite asbestos.  This activity was Class III asbestos work.  The employer did not provide Class III asbestos work training to the employee.  This condition potentially exposed the employee to an asbestos hazard.
