29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use.  [63 FR 33450, June 18, 1998]    a) facility wide- Employees use Laundry Destainer and Detergent but are not provided with a 15 minute continuous eye wash station. The Laundry Destainer has a DOT classification of 8 which is classified as a corrosive and a pH of 12.5. The condition was noted on or about 10/23/13.    NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN ADDITIONAL PENALTY OF $1,000 AS PER 29 CFR 1903.19.

Sufficient access and working space shall be provided and maintained about all electric equipment to permit ready and safe operation and maintenance of such equipment.    a) laundry room- The employer did not ensure that the electrical panel that operates the Chicago Comet folding machine, model EA was accessible in case the machine needed to be shut down or when maintenance needed to access it, on or about 10/23/13. 29 CFR 1910.1200(e)(1): Employers shall develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, material safety data sheets, and employee information and training will be met, and which also includes the following:    a) Facility wide- The employer did not develop and implement a written hazard communication program for employees who are exposed to chemicals such as but not limited to, Laundri Destainer and Detergent. The Laundri Destainer has a DOT classification of 8 which is classified as a corrosive and a pH of 12.5. The condition was noted on or about 10/23/13. 29 CFR 1910.1200(h)(1): Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and material safety data sheets.  a) facility wide- Employees are not trained on the hazards associated with the use of chemicals in their work area or when a new chemical is introduced. Employees use Laundri Destainer which is classified under DOT as a class 8 corrosive with a pH of 12.5 and are not trained on the hazards of this chemical, on or about 10/13/13.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN ADDITIONAL PENALTY OF $1,000 AS PER 29 CFR 1903.19.
