On or about 10/22/2013, at the Port of St. Mary, the employer did not ensure that guards were installed to prevent a thirty foot fall through the skylights.

Or, on roofs 50-feet (15.25 m) or greater in width, each employee was not protected by use of a monitoring system. On or about 10/22/2013, at the Port of St. Mary, the employer did not ensure that fall protection was worn by the employee/s while working on a warehouse roof at an elevation of 30 ft.
